Corneal Parameter Analysis Following Globe-Derived Doughnut Refractive Lenticule Transplantation

Corneal parameter analysis is central to understanding the structural, optical, and biomechanical transformation that follows DRLT using a globe-derived doughnut lenticule. Unlike SMILE-derived lenticules, the globe-derived construct provides a naturally thicker, anatomically contoured stromal profile that can induce more uniform anterior corneal regularization, enhanced cone neutralization, and stable biomechanical redistribution. Post DRLT evaluation through serial Pentacam-based parameters such as K1, K2, Kmax, anterior and posterior elevation, thinnest pachymetry, Q-value, and total corneal refractive power offers an objective map of the progressive remodeling that occurs over weeks to months. This section provides a systematic review of these parameters in patients receiving globe-derived doughnut lenticules, highlighting early optical recovery, mid-term stabilization, and long-term improvements in refractive symmetry. Together, these analyses validate the predictable behavior of the doughnut lenticule within the corneal stroma and underscore its value in managing advanced keratoconus.
