Effective management of shock and hemorrhage in tactical combat settings is paramount, as hemorrhagic shock remains the leading cause of preventable combat fatalities. This chapter details the pathophysiology and classification of hemorrhagic shock, emphasizing the critical importance of rapid field recognition through clinical signs such as altered mental status and weak pulses. It outlines the principles of hemorrhage control across the continuum of care—Care Under Fire, Tactical Field Care, and Tactical Evacuation Care—detailing specific interventions applicable in each phase. Key techniques including tourniquet application, direct pressure, hemostatic agents, and wound packing are thoroughly described. The chapter also stresses the significance of principles of damage control resuscitation including judicious fluid resuscitation, advocating for permissive hypotension and the prioritized use of blood products and whole blood when resources allow. The crucial role of tranexamic acid (TXA) in stabilizing blood clots and improving outcomes is also highlighted. Furthermore, the unique tactical and environmental challenges encountered in combat operations, including specific adaptations relevant to the Indian forces operating in diverse terrains and under varying logistical constraints, are addressed. The chapter concludes by emphasizing the fundamental principles of early and aggressive hemorrhage control, judicious fluid resuscitation, and antifibrinolytic support as critical to improving casualty survival. The necessity of stabilizing casualties in the field and the subsequent management at higher echelons of medical care, such as military hospitals, are also core tenets of our approach.
