Military service demands extraordinary physical and physiological adaptations due to extreme environmental conditions, high-altitude exposure, and combat stress. This chapter explores the anatomical and physiological modifications that soldiers undergo, particularly in high-altitude and cold-weather environments, and their implications for military medicine. Key physiological adaptations include enhanced musculoskeletal endurance, improved cardiovascular efficiency, and optimized pulmonary function to sustain prolonged exertion and oxygen supply. Neurological and psychological resilience, including heightened reflexes and stress responses, play a crucial role in operational effectiveness. The endocrine system supports survival through increased stress hormone production, but prolonged activation can lead to fatigue and metabolic imbalances. Thermoregulation mechanisms help soldiers endure extreme cold, yet they remain vulnerable to conditions like frostbite and hypothermia. Additionally, immune system adaptations provide enhanced defence but expose personnel to unique infection risks in combat zones. This chapter also highlights the medical challenges associated with these adaptations. Conditions such as high-altitude pulmonary oedema (HAPE), high-altitude cerebral oedema (HACE), stress-related disorders, and cold injuries require specialized preventive and treatment strategies. Other common conditions seen in this group include dehydration, heat exhaustion, chronic musculoskeletal injuries, sleep disturbances, nutritional deficiencies, and respiratory infections. Understanding these physiological variations is essential for military doctors to provide effective trauma care and optimize soldier performance in extreme conditions.
