Evacuation to Definitive Care and Airlift Operations

Aeromedical evacuation is a critical link in the combat casualty care chain, enabling rapid, lifesaving transfers from the point of injury to definitive care facilities. This chapter explores the clinical, operational, and physiological aspects of patient transport in both rotary and fixed-wing platforms, with emphasis on tactical decision-making, preflight preparation, equipment requirements, and in-flight management. Through integration of triage scoring systems, like CCATT activation score, this chapter provides a practical framework to determine urgency and platform selection. It also highlights the physiological stressors of flight—such as changes in barometric pressure, gas expansion, hypothermia, and vibration—and their impact on trauma patients. Drawing on military and paramilitary experience in India, the chapter introduces the structure of Critical Care Air Transport Teams (CCATT), the role of Patient Transfer Units (PTUs), and innovations like Quick Conversion Kits (QCK). The chapter emphasizes safety, redundancy in oxygen and power systems, and the training required for high-risk transfers. Case-based learning is applied to illustrate pre-transport stabilization in a casualty with a tension pneumothorax and mangled extremity. In austere and geographically challenging battle zones, the capacity to deliver critical care in-flight is a game-changer—turning distance into survivability.
