Obesity and Depression

Depression and obesity are highly prevalent and frequently co-occur, with evidence suggesting a bidirectional relationship. Epidemiological studies indicate that depression increases the risk of subsequent obesity, while obesity elevates the likelihood of developing depression. Genetic overlap, shared biological pathways, and environmental influences further reinforce this connection. Neurobiological mechanisms underlying their association include dysregulation of the hypothalamic–pituitary–adrenal (HPA) axis, leptin and insulin resistance, neuroinflammation, oxidative stress, and alterations in gut microbiota. These processes interact with psychosocial factors such as stigma, maladaptive eating behaviors, and reduced physical activity, exacerbating both conditions. Chronic low-grade inflammation and metabolic dysregulation have emerged as key contributors to this comorbidity. Inflammatory cytokines influence monoaminergic neurotransmission, neuroplasticity, and stress reactivity, while HPA axis overactivation promotes central adiposity and mood disturbances. Additionally, impaired leptin and insulin signaling may disrupt reward and neurotrophic pathways, further linking metabolic dysfunction with depressive symptoms. Recent findings also highlight the role of the gut–brain axis and immune–metabolic interactions in this relationship. Understanding the complex interplay between depression and obesity is crucial for developing targeted prevention and treatment strategies. Integrative interventions addressing both metabolic and psychological factors may improve outcomes and reduce the burden of these overlapping conditions.
