Sex differences in mental disorders cannot be explained solely by psychosocial factors; biological variables such as brain structure, neuroendocrine function, and reproductive physiology also play a crucial role. These distinctions offer valuable insights into the clinical characteristics of bipolar disorder in women, guiding disease risk estimation and individualized treatment strategies. Women with bipolar disorder tend to experience more frequent depressive episodes, mixed features, and rapid cycling than men. Hormonal fluctuations across the menstrual cycle, pregnancy, and menopause significantly influence mood regulation through estrogen-mediated modulation of monoamine systems. Pregnancy and the postpartum period represent particularly vulnerable phases, with the risk of relapse sharply increasing within the first month after childbirth. Discontinuation of maintenance therapy during pregnancy substantially elevates recurrence risk, underscoring the need for careful treatment planning. Lithium and lamotrigine are regarded as relatively safer treatment options compared with valproate during pregnancy, although risks remain dose- and trimester-dependent and require individualized monitoring. Preconception counseling, therapeutic drug monitoring, and early postpartum prophylaxis are essential for relapse prevention. Understanding sex- and hormone-related factors in bipolar disorder provides a foundation for personalized and evidence-based management of women across reproductive stages.
