Biological Non-pharmacological Treatments for Bipolar Disorder

Bipolar disorder (BD) is a chronic, recurrent psychiatric illness associated with substantial disability and mortality. Pharmacotherapy remains the cornerstone of management; however, a significant proportion of patients demonstrate treatment resistance, poor tolerability, or contraindications to medications. Biological non-pharmacological treatments (BNPTs), therefore, represent crucial adjuncts or alternatives. This chapter reviews the mechanisms, efficacy, safety, and guidelines for electroconvulsive therapy (ECT), repetitive transcranial magnetic stimulation (rTMS), transcranial direct current stimulation (tDCS), deep brain stimulation (DBS), vagus nerve stimulation (VNS), and chronotherapeutic modalities including bright light therapy (BLT) and sleep deprivation. We provide practical checklists, parameter tables, and special-population considerations. While ECT remains the gold standard for severe or treatment-resistant BD, neuromodulatory and chronotherapeutic approaches offer expanding options for bipolar depression, mixed states, and rapid cycling. Future directions include biomarker-guided personalization, protocol optimization, and durable maintenance strategies.
