Bipolar Disorder and Addiction

This review explores the complex relationship between bipolar disorder (BD) and comorbid addictive disorders, including alcohol use disorder (AUD), other substance use disorders (SUD), and behavioral addictions such as gambling and Internet use. BD and AUD exhibit high rates of co-occurrence, which are associated with more severe clinical courses, early onset, increased relapse rates, and poorer treatment outcomes. Genetic, neurobiological, and psychosocial factors are implicated in the shared pathophysiology underlying this comorbidity, with recent advances highlighting genetic markers and neurochemical pathways involved. The clinical management of BD with comorbid addictions remains challenging, requiring integrated pharmacological and psychotherapeutic approaches. Effective treatment strategies should emphasize mood stabilization, reduction in substance use, and relapse prevention, alongside patient education and close monitoring. This review emphasizes the importance of a comprehensive, multidisciplinary approach to improve prognosis and reduce the burden associated with co-occurring bipolar and addictive disorders. Future research should focus on developing tailored treatment modalities based on the underlying neurobiological mechanisms.
