IOL Calculation in Pediatric Cataract

Intraocular lens (IOL) calculation in pediatric cataract surgery is challenging due to the dynamic nature of pediatric ocular growth, difficulties in obtaining accurate measurements, the need for age-specific refractive targets, and the lack of pediatric-specific formulas. Unlike adult cataract surgery, where the goal of IOL calculation is to achieve an emmetropic visual outcome after surgery, the IOL power selection for pediatric cases focuses on ensuring optimal long-term visual development. The aim is to provide these young patients with the best possible vision as they grow, minimizing the risk of amblyopia or avoiding it altogether. Additionally, the risk of refractive surprises and the need for long-term management further complicate the process. Despite these challenges, advances in biometry, surgical techniques, and IOL design are helping to improve outcomes in pediatric cataract surgery.
