Intraocular Lens Power Calculation in Uveitic Cataract

摘要

Accurate intraocular lens (IOL) power calculation in uveitic cataract remains challenging due to chronic inflammation and its effects on ocular biometric parameters, including axial length, anterior chamber depth, keratometry, and lens thickness. This chapter reviews the key considerations for preoperative planning in patients with uveitic cataract, emphasizing the importance of achieving a quiescent eye before surgery, selecting biocompatible IOL materials, and understanding inflammation-related alterations that may affect biometric measurements. The advantages and limitations of various IOL materials are discussed, with hydrophobic acrylic lenses generally considered the preferred option because of their favorable inflammatory profile. The chapter also outlines factors influencing IOL calculation accuracy, compares commonly used IOL formulas such as Haigis, Hoffer Q, and Barrett Universal II, and reviews the role of modern biometric technologies, including optical biometry, swept-source optical coherence tomography, and ultrasound A/B-scan. Practical recommendations and clinical pearls are provided to optimize refractive outcomes and minimize postoperative complications in this complex patient population.
