Current State of Clinical Trials in Urological Cancers and Future Directions

摘要

Since the past decade, clinical trials in urological cancers have undergone a significant transformation, driven by advances in targeted agents, immunotherapy, and precision medicine. This chapter illustrates a wide overview of the present clinical trial landscape across major urological malignancies, including renal cell carcinoma, bladder cancer, and prostate cancer. Recent trials brought to light the emerging role of immune checkpoint inhibitors, antibody–drug conjugates, radioligand therapies, and combination regimens, along with adaptive and platform-based trial methodologies. Emphasis is placed on the evolution of trial designs, therapeutic strategies, and biomarker-driven approaches that have redesigned treatment paradigms and improved patient outcomes. Future directions focus on assimilating real-world evidence, developing personalized treatment strategies and boosting trial designs to accelerate translation of research into clinical practice. The chapter also investigates global and Indian prospects, detecting trends in trial activity, regulatory frameworks, and collaborative research efforts. Despite substantial progress, challenges such as disease heterogeneity, patient recruitment, and equitable access remain.
