Central lumbar spinal stenosis represents one of the most common degenerative conditions affecting the aging spine, with neurogenic claudication significantly impacting the patient’s quality of life. The evolution of minimally invasive surgical techniques has revolutionized the treatment paradigm for this condition. Interlaminar endoscopic decompression, in particular, has emerged as an advanced surgical technique that combines the thorough decompression achieved by traditional open surgery with the advantages of minimally invasive approaches. This chapter provides a comprehensive examination of the interlaminar endoscopic approach for central stenosis, encompassing the fundamental concepts, detailed surgical methodology, and clinical outcomes that define this advanced technique. The interlaminar approach utilizes the natural anatomical corridor between adjacent laminae to access the central spinal canal under direct endoscopic visualization. This technique preserves the posterior tension band while achieving effective decompression of neural elements, representing a significant advancement in the field of minimally invasive spine surgery.
