Transforaminal Endoscopic Discectomy

In treating lumbar disc herniation with nerve root compression, endoscopic lumbar discectomy has gained significant popularity. This rise is driven by advancements in surgical instrumentation, endoscopic equipment, and structured training programs for spine surgeons. Among these techniques, the transforaminal approach, stands out as one of the least invasive options. It offers the flexibility of being performed under local anesthesia, monitored anesthesia care (MAC), or general anesthesia, yielding clinical outcomes in pain relief and quality of life comparable to traditional microscopic discectomy. However, achieving optimal outcomes and minimizing complications requires a through understanding of patient-specific pathology, including strict adherence to indications, contraindications, and procedural limitations. Meticulous preoperative evaluation and precise execution of every surgical step, ranging from anesthesia and patient positioning to the accurate placement of guide wires, obturators, and the endoscope, are critical. Ultimately, comprehensive theoretical knowledge and adequate hands-on training are mandatory prerequisites for surgeons before integrating this advanced technique into clinical practice.
