Techniques and Complication Prevention for cMIS

Adult spinal deformity (ASD) in elderly patients can cause pain, gait disturbance, and reduced quality of life, often requiring surgery when conservative treatment fails. Conventional posterior corrective fusion is highly invasive and associated with substantial complications. Since the 2000s, minimally invasive techniques using lateral lumbar interbody fusion (LIF) and percutaneous pedicle screw (PPS) fixation have reduced surgical invasiveness for ASD treatment. This chapter describes our institution’s experience with circumferential minimally invasive surgery (cMIS) for ASD, including surgical techniques, clinical outcomes, pitfalls, and complication prevention strategies.
