Personalized Medicine in Cancer: Challenges and Future Perspectives

Personalized medicine has improved the way to treat cancer by using very innovative detection and screening methodologies that are quite specific genetically and clinically to each patient’s tumors. Recent advancements in human genome sequencing, proteomics, large analytics, and other advancements in cancer treatment abandon the existing approach in favor of customized treatment that improves the treatment efficacy while reducing toxicity. This chapter offers a complete overview of standard traditional approaches and current methodology in personalized medication in oncology, focusing on next-generation sequencing, liquid biopsy, immunotherapy, and artificial intelligence-based analysis. In this setting, specific approaches that include biomarker-based immunotherapy, patient-mediated organoids, mRNA-based vaccines, and antibody-drug conjugates expand the boundaries in customized cancer therapy. Although there has been significant progress, there are numerous barriers remaining to resolve that hinder the widespread application of personalized medicine such as tumor heterogenicity, clonal development, and cured treatment resistance. Furthermore, legislative obstacles, ethical concerns regarding genomic data privacy, treatment expenses, and disparities in global healthcare infrastructure constantly restrict accessibility and acceptance, especially in environments with limited resources. Expecting future advancements, augmentation of multi-omics data integration, improvement in real-time monitoring by adaptive therapy technologies, and the extension of methodologies for rare pediatric cancer treatment therapy. Reform in policymaking, employment growth and innovative reimbursement mechanisms are very crucial for ensuring economical and accessible healthcare. With the current advancement in technology and clinical settings, personalized medicine promises to enhance the rate of cancer survivors and help in the quality of life globally by administering the right therapy to the right patient at the right time.
