Post-Stroke Unaddressed Complications (Fatigue, Incontinence, Depression, and Pain)

Post-stroke complications such as fatigue, incontinence, depression, and pain significantly affect recovery post-stroke and quality of life. Post-stroke fatigue (PSF) is a common issue, affecting nearly half of stroke survivors. Unlike regular fatigue, PSF is often persistent and does not improve with rest, leading to physical, emotional, and cognitive exhaustion. PSF is assessed using tools like the Fatigue Severity Scale, and management strategies include medications, tailored exercise programmes, energy-saving techniques, and complementary therapies such as mindfulness and yoga. Incontinence, particularly urinary incontinence (UI), affects 38%–60% of stroke survivors shortly after the event, with about 19% experiencing long-term symptoms. Management often involves tailored interventions like timed voiding, pelvic floor exercises, and occasionally, electrical stimulation. Faecal incontinence, also common post-stroke, is addressed through dietary changes, medications, and emerging techniques like transanal irrigation. Post-stroke depression (PSD) is another significant complication, affecting up to one-third of stroke survivors. It hampers recovery, reduces participation in rehabilitation, and lowers overall quality of life. In low-resource settings, tools like the Patient Health Questionnaire (PHQ-9) and telemedicine solutions enable early detection. Treatment involves a combination of medications, simplified cognitive-behavioural therapy, physical activity programmes, and community-driven social support. Pain after stroke is a common issue but remains relatively unaddressed due to an inadequate understanding of pathophysiology and clinical consequences. Pharmacological interventions like tricyclic antidepressants (TCAs), gabapentin, botulinum toxin, and non-pharmacological interventions such as mirror therapy and functional electrical stimulation can be utilized to treat post-stroke pain.
