Multimorbid and chronic diseases are increasingly shaping the healthcare system, bringing with them diverse perspectives, treatment methods, and interactions. To address the challenges of multimorbidity and chronic diseases, concrete measures are necessary at all levels of care. The following chapter explains the concept of multimorbidity and its associated facets. Using the various influencing factors and the societal relevance arising from multimorbidity and chronic diseases in the areas of research, hospitalization, quality of life, and the need for care, the requirements and solution approaches for the multiprofessional treatment team are clarified.
