The aim of this article is to present the fundamentals of knowledge management and knowledge transfer in the context of caring for people with chronic diseases. Knowledge management is explained from the perspective of organizational action, while knowledge transfer is discussed with a focus on the actors involved. Building on this, the relevance of knowledge transfer and efficient documentation in multiprofessional care processes is highlighted. Finally, selected methods of knowledge transfer are outlined to develop approaches for application in chronic care.
