The Handbook Chronic Care in Science and Practice fills the gap left by previous publications by taking an interdisciplinary and interprofessional approach to the care of people with chronic illnesses. The handbook is aimed, on the one hand, at students and teachers of Chronic Care Sciences and their related disciplines, and, on the other hand, at practitioners who wish to broaden their knowledge through adjacent perspectives. Chronic Care is presented as a transdisciplinary field that, independent of disciplinary boundaries, integrates the respective contributions of different disciplines for joint problem-solving and approaches to action.
