Up to a third of all patients undergoing hernia operations have sarcopenia. This chapter describes the definition, diagnosis, and preoperative evaluation of sarcopenia. It discusses the influence of sarcopenia on the risk of developing hernias and the postoperative outcomes following hernia surgery, based on current literature. While evidence indicates a worse outcome for sarcopenic patients after abdominal surgery, this correlation appears less clear following hernia repair. Additionally, prehabilitation is presented as a relevant component for managing sarcopenic patients.
