A variety of behavior change techniquesbehavior change technique support people with diabetes and obesity to change their lifestyle. Based on meta-analyses, measures are proposed which, from the perspective of those affected, are highly effective for a sustainable lifestyle change. In order to maintain a healthy lifestyle, measures that promote the autonomyautonomy of those affected prove to be promising. The challenge of sticking with it in the maintenance phase can be described in the form of a consumer journey. This phase is understood as an iterative process. If the maintenance of a healthy lifestyle is successful, those affected have a high degree of self-management, which is increasingly supported by technical aids that function as an “extended self”. The design of the healthcare ecosystemhealthcare ecosystem also makes a relevant contribution to success. There is potential for optimization in areas such as interprofessionalisminterprofessionalism.
