In pediatric and adolescent rheumatology, one is occasionally confronted with the question of whether the visible and measured inflammation actually corresponds to an immunological-rheumatological disease in the narrower sense. Due to the complexity of the musculoskeletal system, metabolic and structural aspects must also play a role in this differential diagnostic consideration, as for example, subsequent or parallel inflammatory phenomena can occur. These can even show up chronically and in the long term, involving muscles, bones, cartilage, and tendons. Into this tension field, we present a boy with an unrecognized structural disorder in the sense of an epiphysiolysis, which at first glance presented itself as rheumatic coxitis.
