Navicular Stress Fractures of the Foot

Navicular stress fractures occur because of the unique loading of the foot in repetitive injuries. It is often a sport-related injury in younger athletes and can be a career ending injury. Associated issues such as vitamin D deficiency need to be addressed, and this plus training and orthotics may resolve a minimally displaced fracture. However, these patients need to be followed closely if nonoperative treatment is followed because displacement of the navicular fracture can be devastating. If the fracture fails to heal or is caught later, then operative treatment may be required. This can consist of percutaneous screw fixation, open fixation and grafting, and in advanced cases, reduction and grafting. On occasion, a fusion may be required for cases still not healing or with displacement and arthritic change.
