Caregiver burden is a pivotal phenomenon in palliative care, directly impacting the stability of the patient-caregiver dyad, as well as the continuity, safety, and quality of care. Despite its clinical and organizational significance, the scientific literature lacks nursing-sensitive indicators capable of isolating the specific contribution of specialized nursing interventions to caregiver outcomes. Recent evidence demonstrates that nursing interventions focused on the dyad are associated with significant reductions in burden, highlighting their appropriateness as a nursing-sensitive outcome. Building on this evidence, the variation in caregiver burden is proposed as a formal indicator of quality, grounded in a dyadic conceptual framework and supported by validated assessment instruments. This indicator aligns with the principles of continuous quality improvement and with the structure-process-outcome model developed by Avedis Donabedian, offering a structured approach to monitoring outcomes influenced by specialized nursing practice. Integrating this indicator into quality improvement cycles strengthens clinical surveillance, facilitates evidence-informed decision-making, and enhances the delivery of safe, consistent, and person- and caregiver-centered palliative care.
