Objective: This study aimed to identify the challenges and potentialities in the management of care for frail older adults as perceived by health professionals. Method: A qualitative study conducted through a focus group in a municipality in the countryside of São Paulo State, Brazil. Results: The analysis revealed two categories: Challenges in the macro and micro-management of clinical care for older adults and Potentialities in care management for the identification and monitoring of frailty in older adults. Conclusions: The challenges of clinical management of frailty among older adults are multifactorial and interdependent, encompassing aspects from the training of health professionals to the development of public policies that prioritize prevention.
