Objective: To explore the efficacy and safety of modified Huanglian Wendan Decoction combined with conventional western medicine in the treatment of patients with post-stroke depression (PSD) of the liver depression and qi stagnation type. Methods: A total of 100 patients with PSD (liver depression and qi stagnation type) who were treated in our hospital from October 2023 to October 2024 were divided into an observation group (n = 50) and a control group (n = 50) using the random number table method. Both groups received escitalopram and stroke rehabilitation exercises, while the observation group was additionally treated with modified Huanglian Wendan Decoction. After 3 months of treatment, the therapeutic effects between the two groups were compared. Changes in neurotransmitters [5-hydroxytryptamine (5-HT), norepinephrine (NE)], neurotrophic factors [nerve growth factor (NGF), brain-derived neurotrophic factor (BDNF)], depression severity [assessed by the Hamilton Depression Scale (HAMD)], and quality of life [assessed by the Activity of Daily Living Scale (ADL)] were measured before treatment and after 3 months of treatment. Adverse reactions during the treatment period were recorded in both groups. Results: After 3 months of treatment: The total effective rate of the observation group was significantly higher than that of the control group (P < 0.05). The HAMD and ADL scores decreased in both groups, and the scores in the observation group were lower than those in the control group (all P < 0.05). The serum levels of 5-HT, NE, NGF, and BDNF increased in both groups compared with pre-treatment, and the levels in the observation group were higher than those in the control group (all P < 0.05). There was no statistically significant difference in the total incidence of adverse reactions between the observation group (18.00%) and the control group (22.00%) (P > 0.05).Conclusion: Modified Huanglian Wendan Decoction combined with conventional western medicine can effectively improve the clinical efficacy in treating PSD, alleviate depression severity, regulate the levels of neurotrophic factors, enhance patients’ quality of life, and exhibit high safety.
