Failure Mode and Effects Analysis (FMEA) is a methodology for risk and failure prevention which has been known for decades. This method is used, among other things, in the automotive sector and in aerospace. Despite numerous existing research projects, there are still persistent and frequently recurring challenges. In this paper, over 60 FMEA reports created in accordance with the VDA/AIAG manual were examined for deviations from the standard and the most frequent anomalies were analyzed. It became clear that the main challenges lie in the transfer of documents from design to process FMEA, the functional and failure network analysis, and the formulation of functions and causes of failure, as well as the actions to be taken. For this purpose, clusters were created from over more than 300 deviations found and standard challenges were identified. This database is intended to serve as an overview to raise awareness of FMEA to be processed and as input for an failure database to support FMEA.
