Repeat Aorto-Bifemoral Graft Using a Retroperitoneal Approach for Recurrent Symptoms of Lower Extremity Ischemia

A 68-year-old man underwent a repeat aortobifemoral graft in June 2024 for critical limb ischemia following a failed aortofemoral bypass in 2018. The patient had an axillobifemoral graft performed in 2022. Preoperative ureteral stents were placed. He had persistent rest pain in the right foot. One week later, he underwent revision of the graft anastomoses between the right limb of the ABF graft and previously placed axillofemoral graft. A right femoral–tibioperoneal trunk bypass with a ringed PTFE graft with a venous collar for persistent ischemic rest pain in the right foot. His postoperative course was satisfactory. The last arterial Doppler in July 2025 showed mild arterial occlusive disease on the right and moderate arterial occlusion of the left lower extremity.
