Fungal Endophthalmitis

Fungal endophthalmitis is an intraocular fungal infection involving the vitreous and/or aqueous. It is less common than its bacterial counterpart. Fungal organisms can be introduced exogenously (e.g., postsurgical, or via open globe injury) or endogenously (i.e., from the bloodstream). Patients typically present with loss of vision, eye pain, eye redness, and photophobia. Fungal endophthalmitis is a clinical diagnosis, though one often made with supporting imaging, such as B-scan ultrasonography, and laboratory testing, such as fungal cultures and PCR. Treatment involves systemic and local (intravitreal) antifungal agents. Some patients benefit from surgical intervention via pars plana vitrectomy. This chapter will provide a broad discussion of fungal endophthalmitis epidemiology, etiology and clinical presentation, diagnosis, management, and prognosis.
