Fungal Keratitis: Epidemiology, Diagnosis and Therapy

Fungal keratitis presents a significant challenge for ophthalmology globally, primarily in terms of ocular morbidity and potential vision loss. Despite substantial advancements in early detection and improved diagnostic technologies, fungal corneal infections continue to pose a challenge to ophthalmology practices, even in highly specialized eye centers. The unawareness of its risk factors, nonspecific clinical presentation signs, low rates of definitive pathogen isolation, and the lack of specific and effective therapies are the most important contributors to its morbidity and visual impact. The eye is recognized as a fertile site for microbe settlement and infection. Its unique anatomical, histological, and immunological features make it particularly vulnerable to fungal growth. The cornea is especially susceptible to infections due to its exposure to external elements and potential trauma. Several factors contribute to its vulnerability, including its lack of blood supply and a limited immune response during the early stages of infection. Therefore, ongoing and future research and development in this field are essential for overcoming the challenges we currently face. This chapter discusses the epidemiology, clinical manifestations, and updated diagnosis and management strategies of fungal keratitis.
