Se-75-HCAT is a bile acid analogue used for diagnosing bile acid diarrhoea (BAD), which results from excessive bile acid entry into the colon. It is subdivided into three types: type 1 BAD results from disease in, or previous resection of, the terminal ileum; type 2 is idiopathic BAD arising in otherwise healthy individuals, and type 3 occurs in post-surgical patients, especially after cholecystectomy and vagotomy. The percentage whole body retention of Se-75-HCAT over 7 days  after oral administration is used to diagnose BAD. Values of <5%, 5-10% and 10-15% indicate severe, moderate and mild BAD, respectively. There are three mechanisms that result in excess transport into colon: ileal malabsorption, increased enterohepatic recycling frequency and idiopathic increased hepatic bile acid synthesis.
