Preparation for Intensive CBT-E

This chapter describes the preparatory phase of intensive CBT-E for adolescents with eating disorders, a crucial stage to ensure that patients are ready to engage actively in treatment. Admission to all forms of intensive CBT-E requires voluntary participation and a clear commitment to take an active role in the therapeutic process. The procedures in this phase differ between patients who have previously received outpatient CBT-E and those who have not. For those with prior experience, the therapist and patient collaboratively identify factors limiting progress. If these barriers cannot be effectively addressed within the standard outpatient framework, the therapist discusses the need for a more structured and intensive setting, such as intensive outpatient, day-hospital, or rehabilitative inpatient CBT-E. For patients who have not previously received outpatient CBT-E, the preparatory phase typically spans four sessions, with the option to extend it to six sessions for highly ambivalent individuals. It incorporates key elements of Step One in standard outpatient CBT-E, with an emphasis on helping adolescents understand the psychological mechanisms maintaining their disorder and strengthening motivation for change. Therapists adopt a collaborative and engaging stance, highlighting the adolescent’s active role in recovery. Core procedures include real-time self-monitoring of eating behaviours, thoughts, and emotions; development of individualized formulations; collaborative weighing; and structured discussions exploring the advantages and concerns of beginning intensive CBT-E. Personalized psychoeducation further enhances insight and reduces stigma. The chapter also outlines parental involvement, including a dedicated session to inform and engage parents while preserving the adolescent’s sense of autonomy.
