The Post-anesthesia Care Unit: Clinical Stewardship, Systems Leadership, and the Expanding Role of the Anesthesiologist

The Post-Anesthesia Care Unit (PACU) represents a critical juncture in the perioperative continuum, where vigilant monitoring, multidisciplinary coordination, and evidence-based management determine postoperative outcomes. This chapter explores the anesthesiologist’s central role in PACU leadership—encompassing clinical oversight, safety, quality improvement, operational efficiency, and economic stewardship. The anesthesiologist serves not only as a clinician but as a systems leader, educator, and innovator, ensuring that PACU processes align with institutional goals for patient safety, throughput, and fiscal sustainability. By integrating principles of Enhanced Recovery After Surgery (ERAS), data-driven management, and multidisciplinary collaboration, anesthesiologists elevate the PACU from a recovery room to a pivotal engine of perioperative value and patient-centered care.
