It is a fast-paced and clinically rich environment where subtle findings can carry life-altering implications and where timely recognition and decision-making are critical to optimizing outcomes. This chapter presents real cases directly drawn from emergency department encounters, reflecting the nuanced and often unpredictable challenges neurologists face at the bedside. From vague sensory complaints to fulminant seizures, acute confusion, or gait instability, neurologic symptoms demand careful lesion localization, a wide but focused differential, and judicious use of imaging and other diagnostic tools to avoid both under- and over-investigation.
