Groin pain in athletes is a frequent and complex condition with multifactorial causes and overlapping clinical features. It is most prevalent in high-intensity sports such as soccer. The 2014 Doha Agreement established standardized terminology, defining four primary clinical entities—adductor-related, pubic-related, inguinal-related, and iliopsoas-related groin pain—along with hip-related and other causes. MRI is the imaging modality of choice, providing detailed evaluation of the pubic symphysis, aponeurotic complexes (P-PAC and PLAC), and associated musculotendinous structures. Recognition of characteristic MRI signs and correlation with dynamic ultrasound findings improve diagnostic accuracy. A multidisciplinary approach integrating imaging, physiotherapy, and interventional management optimizes recovery and return-to-play outcomes.
