HS in Skin of Color

Hidradenitis suppurativa (HS) is a chronic inflammatory skin condition that develops in intertriginous skin, such as the axillae, groin, buttocks, perineum, and inframammary regions. HS can present as abscesses, follicular papules/pustules, cysts, sinus tracts, and scarring. These lesions may appear like the lesions seen in folliculitis, carbuncles, cutaneous Crohn’s disease, and pilonidal cysts. In darker skin tones, HS may present as violaceous/dark brown or erythematous papules and nodules, keloid scarring, or as post-inflammatory hyperpigmentation. Darker skinned patients are three times more likely to develop HS compared to their Caucasian counterparts; however, the cause is unknown. Risk factors for the development of HS include metabolic syndrome, smoking, depression, and a lower socioeconomic class.
