External snapping hip syndrome (ESHS) is an audible or palpable snapping sensation often caused by the iliotibial band (ITB) or the gluteus maximus tendon, when passing over the greater trochanter of the femur during the flexion–extension cycle of the hip during movement. Altered hip biomechanics caused by tightness of the ITB, increased femoral anteversion, imbalances between the hip abductors and adductor muscles, and poor lumbopelvic control might predispose to ESHS. Patients present with snapping, lateral hip pain, and functional limitations. The diagnosis is made mainly by clinical examination. First-line treatment of ESHS is non-operative focusing on symptom relief and biomechanical corrections. Although often self-limiting, surgical management might be required in recalcitrant cases involving either open or arthroscopic release of the ITB in combination with greater trochanter bursectomy. Surgery might be favorable, with high patient satisfaction and return to activity rates.
