Most systems used to describe the appearance of the anterior chamber angle, such as the Shaffer system, use only one characteristic as the basis for grading. This is analogous to have a language with only one letter. Described in this chapter are the essential characteristics of the anterior chamber angle that must be considered in order to describe it accurately and adequately. Also described is the easily-mastered coding method that allows documenting these essential characteristics accurately and quickly. (When an ocular examination reveals a deep anterior chamber, the examiner may conclude that the anterior chamber angle is healthy).* That conclusion may be correct. Importantly, however, that conclusion may be wrong, because it is merely a guess; that guess may result in a person becoming blind unnecessarily. The 2 minutes it takes to gonioscope competently and record the findings is often the start to saving a person’s sight.
