Scoliosis is a 3D process, yet the mainstay of modern management remains the well-validated evaluation of 2D, anterior-posterior, and lateral x-ray films. Existing classification systems are reviewed to provide a framework for the basic measurement guidelines and definitions. Defining a “landmark vertebra” can be helpful in consistently labeling vertebral anatomy when transitional vertebrae or segmentation/formation anomalies exist.
