Imaging in Thyroid Diseases and Cancer

This chapter offers a comprehensive review of imaging in the evaluation and management of thyroid cancers. Ultrasound remains the cornerstone modality for diagnosis, aided by risk stratification systems such as Thyroid Imaging Reporting and Data System and American Thyroid Association guidelines to guide biopsy and surveillance strategies. Cross-sectional imaging with computed tomography (CT), magnetic resonance imaging, and positron emission tomography/CT provides crucial information on extrathyroidal extension, lymph node involvement, distant metastases, and disease recurrence. Challenges in distinguishing benign from malignant lesions, as well as imaging pitfalls and mimics, are examined in detail. Advances, including elastography, contrast-enhanced techniques, artificial intelligence, and molecular testing, hold promise for improving diagnostic accuracy and multidisciplinary decision-making. By summarizing imaging findings across thyroid cancer subtypes, this work emphasizes a patient-centered, evidence-based approach that optimizes outcomes and guides personalized treatment planning at times guided by personal proven experiences or observations.
