The Role of the Advanced Practice Provider in the Management of Patients with Heart Failure

Heart failure (HF) is a chronic, progressive condition that affects millions of people worldwide and requires a multidisciplinary team (MDT) approach to provide quality care and improve patient outcomes. Advanced Practice Providers (APPs) have an integral role in the HF team, through all stages of the disease including optimizing guideline directed medical therapies (GDMT), advancing therapies such as orthotopic heart transplantation and mechanical circulatory support. When HF progresses beyond medical and advanced therapies, APPs are experts at empowering patients to make informed decisions regarding their goals of care. APPs provide access and ensure continuity of care in all settings using a holistic approach that encourages a partnership between patients and providers.
