This paper presents OMNIAHEARTCARE, a modular, scalable digital health system designed to support the integrated management of Chronic Heart Failure (CHF). The platform consolidates data from heterogeneous sources—including Electronic Health Records (EHRs), Personal Health Records (PHRs), and wearable sensors—into a unified architecture. Central to this system is the Virtual Individual Model (VIM), a self-adaptive software module that synthesizes structured and unstructured data to provide holistic, personalized patient profiling. The platform integrates big data analytics and a Decision Support System (DSS) to assist clinicians with early diagnosis, risk assessment, and therapy optimization. Developed using user-centered and ethically grounded methodologies, OMNIAHEARTCARE aims to enhance care continuity and support proactive interventions for elderly and multimorbid populations. This paper describes the system architecture, development process, and ongoing testing strategy, laying the groundwork for future clinical validation.
