Though abdominal pain may be a harbinger of an obstetric complication, pregnant women can also develop abdominal pain unrelated to the pregnancy itself. As many as 1 in 500 pregnant women develop an acute abdomen, and up to 2% of women need an operation during pregnancy for a nonobstetric problem (Diegelmann, Emerg Med Clin North Am 30(4):885–901, 2012; Barber-Millet et al. Cir Esp 94(5):257–265, 2016; Traweek et al. Surg Clin North Am. 103:1217–1229, 2023). Diagnosing the cause of abdominal pain may be difficult, however, due to anatomic and physiologic changes that occur during pregnancy. The gravid uterus may displace abdominal organs, peritonitis may not develop due to abdominal musculature stretching, and leukocytosis is unreliable due to leukocytosis of pregnancy (Barber-Millet et al. Cir Esp 94(5):257–265, 2016; Jones et al. BMJ 345:e6818, 2012). Causes of abdominal pain can range from gynecologic to genitourinary to gastrointestinal.
