Heart failure (HF) is a complex clinical syndrome characterized by a decrease in cardiac output that results in a reduced ability to meet the metabolic demands of the body. It is common and often complicated by multiple co-morbidities and polypharmacy. The cornerstone of treatment is largely driven by medications, and hence pharmacists play an important role in caring for these patients. Initiation and titration of medication are key activities when treating patients with HF, as is follow-up and monitoring.
