Hypertension Assessment

Hypertension is generally a chronic condition associated with vascular damage as a result of sustained high blood pressure, leading to an increased risk of cardiovascular disease, among others. Pharmacists play a role in assessing, educating, and managing patients at risk for and with hypertension. An appropriate patient assessment process will allow pharmacists to optimize therapy for those patients on antihypertensive medications, both during initial start and follow-up. Accurate blood pressure measurement technique is key for informing the assessment and treatment strategy. Since treatment goals vary based on patients’ level of risk, pharmacists can counsel patients on appropriate targets and treatment strategies. Monitoring patient response to medication therapy provides an opportunity to assess both efficacy and safety and improve adherence.
