Working effectively within a team allows the primary care provider (PCP) the potential to manage behavioral health (BH) conditions more proficiently, diagnose more accurately, develop care plans, and follow up at appropriate intervals. PCPs should consider the function and strengths of different members of the office staff within the primary care office and those who may be affiliated with, but also work outside the office. Conceptualizing your team in a “tiered” model may be helpful in considering how you might effectively utilize different team members in addressing different BH concerns of patients. This tiered approach considers team members that act directly with the PCP and patient (first tier), including the patient’s social support, along with those that have a less direct or more distant role (Second and Third Tiers). The provider might maximize their team members beginning with the closest to most distant resources.
