Kinematic Classification

The kinematic classification is based on the analysis of the dominant motor problem: postural control in tetraplegias, walking problems in diplegias, and use of the affected hand in hemiplegias. Among tetraplegias, four different postural aspects can be distinguished: the apostural condition, akinetic form characterized by global flexion patterns, antigravity with horizontal trunk, in the variant dominated by startle and the one dominated by propulsive reaction, and finally antigravity with vertical trunk, which also characterizes all diplegias and hemiplegias cerebral palsy (CP) forms. By analyzing walking problems of diplegic children, at least four forms can be distinguished: propulsive, tight skirt, tightrope walkers, and daredevils. In addition to these forms, double hemiparesis is described. The affected hand of hemiplegic children is described in five different situations: integrated, semifunctional, synergic, prisoner, and excluded. The kinematic classification is useful not only to understand the natural history of palsy and to predict functional prognosis, but above all to understand which therapeutic interventions are more effective for CP child recovery among physical therapy, physiotherapy, focal, district, or systemic drugs, orthopedic and neurological surgery, orthoses, and aids.
