While clinical evaluation is fundamental, imaging techniques are crucial in detecting pathology and guiding decision-making in endodontics. The transition from radiographs to CBCT has enhanced endodontic diagnosis, particularly for molars with complex anatomy. However, given CBCT’s higher radiation dose, limitations derived from imaging artefacts and absence of soft tissue depiction, diagnosis is sometimes compromised. MRI has emerged as a promising alternative to X-ray-based imaging in endodontics, offering non-ionising examination, including soft-tissue visualisation. Integrating evidence-based imaging protocols into routine practice is essential to enhance diagnostic accuracy while prioritising patient-centred care and minimising radiation exposure. This chapter details the role of imaging in endodontics, with a particular focus on molars.
