Therapeutic Alliance and Rupture Repair with Children in CBT: Creating Better Clinical Chemistry and Catalyzing Curiosity

This chapter offers clinicians key tips in achieving better treatment partnerships between their young patients, caregivers, and themselves. In order to set the stage for this clinical alchemy, the literature examining therapeutic relationships and alliance rupture repair in CBT with young patients is briefly reviewed. Often, alliance ruptures are more subtle and nuanced with youth. Moreover, alliance rupture repair requires flexibility, creative, and experiential interventions. The existing research findings are synthesized yielding a formula for better interpersonal chemistry in CBT for addressing ruptures. The nine pivotal elements in this recipe include scaffolding a case conceptualization, establishing prototypical session structure, igniting collaboration, implementing proper pacing through the session, practicing with fidelity and flexibility, demonstrating informality, building artful Socratic dialogues, embracing inevitable alliance ruptures, and intervening in here-and-now moments. Finally, confabulated case examples and therapeutic dialogues emphasize the salient points.
