Liver transplantation is the only established solid organ transplant that can be used to treat certain malignancies. Conditions such as hepatocellular carcinoma, intrahepatic or perihilar cholangiocarcinoma, colorectal liver metastases, and some rarer cancers may be considered for this procedure. Proper patient selection is crucial and involves evaluating factors such as curability, transplantability, resectability, and operability-all tailored to the specific underlying disease. Along with advancements in oncological systemic and locoregional treatments, we discuss current pretransplant considerations, including downstaging and bridging, as well as future opportunities for expanding liver transplantation as a viable treatment option for these patients.
